Submitting an appeal application

If you would like to appeal a decision made by the Social Security Department, please ensure you have asked the Social Security Department for your claim to be reviewed by a different officer or medical board before submitting your appeal. If you are still not satisfied after this redetermination, please complete and submit the appeal form below, enclosing a copy of your redetermination letter.
